Timothy Ray Mayfield, 41, of North Pine Street, a plumber and pipefitter, died Thursday at home. 

Mr. Mayfield was born in Fort Eustis, Va., and moved to this area in 1962 from Toano, Va. He was a member of Plumber and Pipefitters Union 295. 

Survivors include his wife, Susan Peterson; his mother, Emma Phippins, and a brother, Richard Phippins, both of Bunnell; two sisters, Cathy Azallion, Pasadena, Md., and Marjorie Phippins, Bunnell; and his grandmother, Jessie Barrow, Bunnell.
